Programmed Health Professionals has over 35 years of experience in assisting and supporting clients living with disability, as well as those who have sustained spinal cord injuries, acquired brain injuries and multiple other traumas following motor vehicle and workplace injuries. Recognising the unique requirements needed to assist clients and their families to make independent and informed life choices, our staff will work with you to develop an individual support plan tailored to your needs.

As an approved provider to the National Disability Insurance Agency (NDIA), Programmed Health Professionals is eligible to provide support to National Disability Insurance Scheme participants.


We can assist you with:

Your transition from hospital/rehabilitation to home

Our relationship with you begins in the hospital/rehabilitation centre. A personally appointed care manager will visit you and your family before you’re discharged, ensuring a smooth transition from hospital to home.

Your care team

The team appointed to your Individual Support Plan have specific training and expertise in such speciality areas as acquired brain injury, spinal cord injury, burns, multi system trauma, rehabilitation and case management.

Our disability support staff have an average tenure of six years, meaning you can be assured of a regular and consistent team of health professionals who will work with you and your family on an ongoing basis. Your Care Manager is available 24 hours a day 7 days a week.

Choosing your care team

We encourage you to meet prospective members of your care team prior to commencement of services. We recognise how important it is for you to be absolutely comfortable with those involved in your Individual Support Plan.

Regular feedback

Programmed Health Professionals encourages all clients and their families to meet on a regular basis to discuss progress of the Individual Support Plan.

At any time you can ask to speak confidentially with your Care Manager if you wish to provide any feedback about your care team.
